In Italy, gambling is governed by the Gambling Commission that was established in 1963. It is responsible for regulating lotteries and other forms of gaming. The commission has regulatory authority over gambling operators, lottery organizers and sports betting agents which operate in Italy. The commission's responsibilities include ensuring that all these organizations comply with the provisions of relevant laws and regulations governing their activities; issuing licenses to operators who demonstrate good faith intention to abide by the law; organizing public awareness campaigns for consumers about how to identify illegal providers of services online; penalizing non-compliant or rogue entities through fines or even revocation of license; coordinating enforcement actions with other agencies including police forces, tax authorities and judicial bodies throughout Italy.
The commission issue its license only for the local market. It follows that the casinos with the Italian license can not operate internationally (given that they don't have any other licenses).
